The National Health Service Corps is a federal program that pays up to $75,000 in tax-free loan repayment to nurse practitioners and other primary care clinicians who work at approved sites in Health Professional Shortage Areas.
What it means in plain English
The National Health Service Corps is run by HRSA (the Health Resources and Services Administration) and offers two main pathways: the NHSC Loan Repayment Program (LRP) and the NHSC Scholarship Program. For practicing NPs, the LRP is the relevant track.
The standard NHSC LRP award is $50,000 in exchange for two years of full-time service (32+ hours per week) at an approved site, or $25,000 for half-time service. The Substance Use Disorder Workforce LRP and the Rural Community LRP versions extend awards up to $75,000 for the same two-year commitment.
Approved sites must be located in Health Professional Shortage Areas (HPSAs) and be designated NHSC sites. Most Federally Qualified Health Centers, rural health clinics, Indian Health Service facilities, and many community mental health centers qualify.
Why it matters for NP students
NHSC payments are tax-free at the federal level, which makes the effective value of a $75,000 award closer to $100,000 to $115,000 of pre-tax compensation. For an NP earning $115,000, that's almost a year of post-tax income.
NHSC service also counts as qualifying employment for PSLF, so you can stack PSLF credit with NHSC repayment. Two years of NHSC equals 24 of your 120 PSLF payments while $75,000 of debt is independently retired.
Many NHSC-approved sites are exactly the kind of underserved community where new NPs can practice at the top of their license, see complex patients, and grow clinical skills faster than in suburban primary care.
How it actually works
The math behind National Health Service Corps is more concrete than most borrowers realize. Here's a worked example using current 2026 numbers.
Common pitfalls
- Accepting a job at a non-NHSC site and assuming the LRP will still apply.
- Dropping below 32 hours/week, which voids the full-time commitment.
- Treating NHSC service months as automatically counting for PSLF, they do, but only if you are also enrolled in an IDR plan and certifying employment annually.
- Underestimating the rural lifestyle adjustment if you choose a remote HPSA site.
- Letting the application deadline pass, NHSC LRP cycles open and close on a fixed annual calendar.
